Every PA type's General Information tab contains a common set data sections plus either Hospital, a Referring Provider, an Ordering Provider, or a Prescribing Provider section. One of these four sections is displayed depending on the PA type.

Client Information

The Client Information section displays client demographic (e.g., Client ID and Name) data.

Prior Approval Response

This section gives you basic information on DOH's response to your request. The section contains fields such as Issue Date and Effective Date. A PA Number is not eligible for submission or billing until the PA is Approved. If the term 'Not for Billing' appears next to the PA number, it is not eligible for billing and should not be submitted for processing.

Requesting Provider Contact

The common Requesting Provider Contact section contains demographic data about the request's contact person such as Name and Telephone.

Requested Service

The General Information tab contains a common Requested Service section containing fields such as Service Type Code and Order Date.

Diagnosis

This section contains diagnosis codes, if any, intended to justify the request.

Comments

The General Information tab contains a common Comments section containing free form text intended to clarify the request.

The response detail page ends with a Close button that returns you to the Prior Approval Activity Worklist page. If you are viewing a PA that has never before been viewed, a Worked button also appears at the bottom of the page. Choosing this button also returns the Prior Approval Activity Worklist page, but sets the Provider Status to Viewed.
