Ordering Provider: DME/Supplies, PDN, Physician

The General Information tab on PA response pages with DME Supplies, PDN, Physician PA types has an Ordering Provider section. This section contains demographic data such as Name and State License # about the provider who ordered or will order the equipment, supplies, or service.
