State License Number

When entering the State License Number for a Provider in ePACES, there are strict rules and formatting that must be followed. The basic format for the value is: 3-digit Profession Code + 2 character State Code or Privilege Code + 6 digit license number equaling an 11-digit value.

Profession Code

• The Profession Code that is entered must begin with a Zero. Below is a list of valid profession codes:

Privilege Code Values

• If entering a NYS Provider, the qualifier should be '00'.

• If entering an Out-of-State Provider, the qualifier should be the 2-character state code. For example, a Provider located in New Jersey would have a qualifier of NJ.

• NYS Nurse Practitioners who are allowed to prescribe will have a qualifier of '0F'.

• NYS Optometrists who are allowed to prescribe will have a qualifier of '0V' or '0U'.

State License Numbers

When entering a State License Number, the last six positions of the entered value should be the actual numeric license number. If the license number does not contain six digits, zero fill the appropriate positions preceding the actual license number.

For example, an Optometrist in New York with a Profession Code of 056 whose State License Number is V867 would be entered as: 0560V000867.
